I have the offense facing the defense (we never turn ouor backs against the enemy) with the center in the middle, hands on thighs with the guards flanking him and the tackles flanking the guards. Behind the line I have the split ends behind the tackles and the slots next to the split ends with the FB in the middle. The QB stands in front calling the plays.

I have tried the old fashioned circles huddle, but I have found the "two line" huddle that Coption describes to be far more effective, gets everyone out faster, and allows your line to come out without having to turn around. Especially in Youth ball, we would get kids confused in the circle huddle as to where to be.
